Your First Visit

When you visit our office for your initial appointment, it will consist of a consultation explaining your diagnosis and treatment options.  Dr. Porchia James will explain your diagnosis and treatment options. Occasionally, surgery can be performed the same day as the consultation, but, some dental insurances have guidelines which restrict same day treatment.  However, a complex medical history and or treatment plan will require a dental insurance pre determination evaluation. We are not in network with any medical insurance companies and we do not accept and/or bill to secondary insurances.

Please assist us by attaching the following information to your electronic history form before your consultation:

  • Attach ID (Driver License, Passport, School ID etc.)

  • Attach dental insurance card front and back.  We are not in network with any medical insurance companies.

  • Attach your referral slip and any x-rays if applicable.

  • Attach medication list if applicable.
