Your First Visit

INITIAL CONSULTATION

Your initial appointment at About Face Oral & Facial Surgery will include a consultation with Dr. Porchia James, who will review your diagnosis and discuss your recommended treatment options. In some cases, surgery may be performed on the same day as your consultation. However, certain dental insurance plans require prior authorization or a pre-determination before treatment can be performed. Patients with a complex medical history or treatment plan may also require additional evaluation, medical clearance, and/or dental insurance pre-determination before surgery can be scheduled. Important Insurance Information: We are not in-network with any medical insurance companies. We also do not accept or bill secondary insurance plans.

CONSULTATION & PANORAMIC X-RAY FEES

If your dental insurance does not cover your consultation, the consultation fee is $110.

If your dental insurance does not cover your panoramic X-ray, the fee is $140.

If you have a panoramic X-ray from another dental office, we may accept it if it meets all of the following requirements:

The panoramic X-ray must be clear and legible.
It must be dated less than one year before your first visit to About Face Oral & Facial Surgery.
It must include the patient's first and last name and date of birth.

If the panoramic X-ray does not meet these requirements or is not adequate for diagnostic purposes, a new panoramic X-ray will be required at our office and the $140 fee will apply.

We strongly recommend contacting your dental insurance company or your employer's Human Resources department before your appointment to verify your dental benefits, coverage, deductible, and any applicable out-of-pocket expenses.

INFORMATION REQUIRED BEFORE YOUR CONSULTATION

Photo ID — Driver's license, passport, school ID, or another valid form of identification
Dental insurance card — Clear photos of the front and back

Please email the following documents to info@aboutfacesurgery.com:

Referral letter, if applicable
Panoramic X-ray dated less than one year ago
Current medication list

APPOINTMENT, PAYMENT & CANCELLATION POLICY

Consultation Appointments: If you cancel, reschedule, or fail to attend your scheduled consultation, an additional $50 fee will apply.

Surgery Scheduling & Payment: Payment is required before a surgery appointment can be scheduled. Patients with an outstanding balance cannot be scheduled for surgery until the balance has been paid in full.

Third-Party Financing: If you are using third-party financing, all financing arrangements and funds must be secured with your financial institution at least one week before your scheduled surgery date.

Surgery Cancellation, Rescheduling, & No-Show: If you cancel, reschedule, or fail to attend your scheduled surgery appointment, an additional $200 fee will apply.

Down Payment Refunds: If you request a refund of your down payment, a $150 processing fee will apply to each refund transaction.

Down Payment Expiration: Down payments must be applied toward treatment within the calendar year in which they are received and cannot be transferred or carried over to the following year.

Appointments Required: We do not accept walk-ins. All consultations, procedures, and other office visits are by appointment only.

IMPORTANT: Outside food, drinks, and deliveries are not allowed in our office.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the consultation and surgery visit.

A pre-operative consultation and physical examination are mandatory for patients undergoing IV anesthesia for surgery. Please have nothing to eat and or drink after midnight. You will also need an adult driver with a valid U.S. government issued driver license to drive you home. If your driver has an expired driver license, we will have to reschedule your appointment. Please advise driver that he or she can not leave premises during surgery. Sedated patient can not operate vehicle for 24 hours. Please alert the office before surgery if you have a medical condition that may be of concern (e.g., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (e.g., heart medications, aspirin, anticoagulant therapy, etc.).

X-Rays

If your referring dentist or physician has taken a recent (less than one year) panoramic X-ray or Cone Beam CT scan, you may request that they email to info@aboutfacesurgery.com. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our office. Also, remember not all x-rays are covered via dental insurance, because of guidelines added per your employer. All X-ray systems in our office are advanced and fully digital.
